From where can I get NIOS admission form for admission in 12th class? What is the last date for submission of application form? What important documents are to be submitted along with the application form?

Eligibility for NIOS admission:
Learners who have already passed Sr. Secondary from recognized Board/University and want to admission in one subject or up to 4 subjects for updating their qualification.
Minimum Age: 16 (Sixteen) years on the date of application.
Document Requirment:
Attested copy of the Certificate Of Matriculation/Secondary Examination having a record of the Date of Birth.If passed Class 12th , then attested copy of the Marksheet/Certificate of Class 12th.

I have filled the form of 10th class from N.I.O.S. on the 28th,JUL 2016 my Name is Harvinder singh my Reference no. A0716222514. But after 5 months they are saying ," your form has rejected due to Require Documents ," But so many child have filled with me but why only my admission has rejected. please help me. kindly provide examination in same year otherwise I lost my one year. .
