Can you provide me the Kanpur university PhD application forms .What the last date to submit the application form is?

Kanpur University is inviting the applications from the eligible candidate for admission in PhD courses. The application form can be downloaded from the official website. For your help I am uploading the file for the application form here.
The application form will be submitted till 31st December 2012.
